Alexander is a wonderful swim coach, the best we've had for either of our two boys (ages 3 and 7)! He has a natural gift in working with children, with a personality akin to a favorite uncle or older cousin. He challenges them and teaches them new skills while laughing and having fun. By submitting this form, I acknowledge that swimming activities involve inherent risks including but not limited to drowning, near-drowning, and physical injury. I voluntarily enroll my child in the Odyssey Swim Program operated by Odyssey Early Learning & Development, LLC, and agree to assume all risks associated with participation.

I authorize program staff to provide emergency medical care if necessary and release Odyssey Early Learning & Development, LLC, its coaches, instructors, and agents from liability for any injury, illness, or loss arising from participation, except in cases of gross negligence or willful misconduct.
